20 minutes before the scheduled game time, please fill out the game card section that reflects your team status. The Home team coach shall fill out the home team side of the card with the minimum of the player's name, uniform number and the coach's signature at the bottom. Upon completing their section, hand the card to the Visiting team coach for them to fill out the visitor side of the card. Once the visiting team has filled out their portion, they shall give it to the referee prior to the equipment check time.

The game card will be administered by the referee and will reflect the official record of the game. The card will be reviewed by the Head Referee and officials if there are red cards and/or yellow cards issued.

Useful Hint : Experienced coaches save time & last minute pre-game panic by preparing a printed stick-on label with all player names & jersey #s to simply stick on the game card. Then simply cross off any teammates not playing that day.
